Redemption) with your name, shipping address, telephone number, e-mail and
sales order numbers from past purchases on
Manic Panic’s® Online Store.
2) List the item(s) you wish to redeem with your Manic Panic® Frequent Dyer Miles™ and please include
Item Number and
Item Description.
