Rules:

2. To redeem your Manic Panic® Frequent Dyer MilesTM you must:
• E-mail manicpanicnyc@aol.com (subject: Redemption) together with your name, address, telephone number, e-mail and sales order numbers from past purchases on www.manicpanic.biz.
• List the item(s) you wish to redeem with your Manic Panic® Frequent Dyer MilesTM. Include Item Number and Item Description.
• You will receive an e-mail confirmation within 2 business days. If you do not please call us toll free at 1 800 95 manic.
3. Redeemed products will be shipped via USPS at Manic Panic’s® expense within 10 working days.
4. Manic Panic® Frequent Dyer MilesTM have no cash value are not transferable.
5. Manic Panic’s® Frequent DyerTM Program is available in the United States only.
6. Manic Panic’s® Frequent DyerTM Program is effective starting November 25, 2004.
